GASTRO-INTESTINAL DISEASE

Gastro-intestinal Disease

Gastrointestinal (GI) diseases encompass a wide range of conditions affecting the digestive system, including the esophagus, stomach, intestines, liver, and pancreas. These disorders can significantly impact daily life, causing symptoms such as abdominal pain, bloating, diarrhea, constipation, and nausea.

Common Gastrointestinal Disorders

  1. Gastroesophageal Reflux Disease (GERD): Characterized by chronic acid reflux, GERD leads to heartburn, chest pain, and regurgitation. It results from the backflow of stomach contents into the esophagus due to a weakened lower esophageal sphincter. Management includes lifestyle modifications, medications like proton pump inhibitors, and, in severe cases, surgery.
  2. Irritable Bowel Syndrome (IBS): A functional GI disorder causing symptoms like abdominal cramps, bloating, and alternating diarrhea and constipation. The exact cause remains unknown, but stress and diet can trigger symptoms. Treatment focuses on dietary changes, stress management, and medications to alleviate symptoms.
  3. Inflammatory Bowel Disease (IBD): Includes Crohn's disease and ulcerative colitis, conditions characterized by chronic inflammation of the gastrointestinal tract. Symptoms encompass persistent diarrhea, abdominal pain, fatigue, and weight loss. Treatment may involve anti-inflammatory drugs, immune system suppressors, and sometimes surgery.
  4. Peptic Ulcer Disease: Caused by the erosion of the stomach lining or the upper part of the small intestine, leading to sores or ulcers. Symptoms include stomach pain, bloating, and indigestion. Treatment typically involves antibiotics for H. pylori infection and medications to reduce stomach acid.
  5. Celiac Disease: An autoimmune disorder where ingestion of gluten damages the small intestine lining, leading to malabsorption of nutrients. Symptoms vary widely and can include diarrhea, weight loss, and fatigue. The primary treatment is a strict, lifelong gluten-free diet.

Diagnosis and Treatment

Diagnosis of GI diseases often involves a combination of medical history review, physical examination, laboratory tests, imaging studies, and endoscopic procedures. Treatment strategies are tailored to the specific condition and may include dietary modifications, medications, and in some cases, surgical interventions.

Conclusion

Gastrointestinal diseases are prevalent and can range from mild to severe. Early diagnosis and appropriate management are crucial for improving quality of life and preventing complications. If you experience persistent digestive symptoms, consult a healthcare professional for evaluation and personalized treatment.

HERNIA

Hernia

A Hernia occurs when an internal organ or tissue bulges through a weak spot in the abdominal wall, often causing pain or discomfort. While some small, asymptomatic hernias may be managed conservatively, surgical intervention is typically recommended for symptomatic or complicated cases.

Surgical Options

  1. Open Hernia Repair: This traditional method involves making a single incision at the hernia site to reposition the protruding tissue and reinforce the abdominal wall, usually with a synthetic mesh. It's effective for various hernia types but may require a longer recovery period.
  2. Laparoscopic (Minimally Invasive) Repair: Utilizing several small incisions, this technique involves inserting a camera and instruments to repair the hernia. It offers benefits like reduced pain, shorter hospital stays, and quicker recovery. Laparoscopic repair is often preferred for bilateral or recurrent hernias.
  3. Robotic-Assisted Surgery: An advanced form of laparoscopic surgery, robotic-assisted procedures provide enhanced precision and control. The surgeon operates robotic arms while viewing a 3D magnified image, improving outcomes, especially in complex or large hernias.

Conservative Management

For small, asymptomatic hernias, a "watchful waiting" approach may be advised. This involves monitoring the hernia for any changes or worsening symptoms over time. Lifestyle modifications, such as avoiding heavy lifting and maintaining a healthy weight, can help manage symptoms.

Conclusion

The choice of treatment depends on the hernia's type, size, and location and the patient's overall health. Consulting with a healthcare professional is essential to determine the most appropriate treatment plan.

UROLOGY

Urology

Urology is a medical specialty focusing on the diagnosis and treatment of conditions affecting the urinary tract and male reproductive organs. Urologists manage a wide range of disorders, including urinary tract infections (UTIs), kidney stones, benign prostatic hyperplasia (BPH), prostate cancer, erectile dysfunction, and incontinence.

Common Urological Treatments

  1. Medications: Alpha-blockers and 5-alpha-reductase inhibitors are commonly prescribed to manage BPH symptoms by relaxing prostate muscles and reducing gland size.
  2. Minimally Invasive Procedures:
    • Transurethral Microwave Thermotherapy (TUMT): Utilizes microwave energy to shrink prostate tissue, alleviating BPH symptoms.
    • Prostatic Artery Embolization (PAE): A non-surgical technique that blocks blood flow to the prostate, leading to its shrinkage.
  3. Surgical Interventions:
    • Transurethral Resection of the Prostate (TURP): A standard procedure for BPH involving the removal of prostate tissue through the urethra.
    • Prostatectomy: Surgical removal of all or part of the prostate gland, often performed for prostate cancer.
  4. Neuromodulation Therapy:
    • Percutaneous Tibial Nerve Stimulation (PTNS): A minimally invasive procedure that stimulates nerves controlling the bladder, effective for overactive bladder symptoms.

GENERAL SURGERY

General surgery

General surgery is a medical specialty that focuses on surgical treatment of conditions affecting the abdomen, digestive tract, skin, soft tissues, and endocrine system. It includes both elective and emergency procedures, often performed using advanced techniques such as laparoscopy (keyhole surgery) and robotic-assisted systems.

Common General Surgical Treatments:

  • Appendectomy: Surgical removal of an inflamed appendix, commonly due to appendicitis. This is often an emergency procedure.
  • Cholecystectomy: Removal of the gallbladder, typically performed to treat gallstones or gallbladder infection.
  • Hernia Repair: Fixes abdominal wall hernias using sutures or surgical mesh, done through open or laparoscopic techniques.
  • Bowel Resection: Removal of a diseased part of the intestine, often necessary for cancer, Crohn’s disease, or obstruction.
  • Mastectomy: Surgical removal of one or both breasts, usually as a treatment for breast cancer.
  • Thyroidectomy: Partial or total removal of the thyroid gland to treat thyroid disorders or cancer.

Modern Techniques:
Minimally invasive surgery, such as laparoscopic and robotic surgery, uses small incisions, reducing recovery time, pain, and hospital stay. These approaches are increasingly preferred due to lower risk of complications.

Postoperative Care:
Recovery varies by procedure but usually includes pain management, wound care, mobility exercises, and follow-up visits. Lifestyle changes and medications may be recommended to support long-term healing.

STONE DISEASES

Stone Diseases

Stone diseases, particularly kidney stones, require treatment based on the type, size, and location of the stone, as well as the severity of symptoms. Small stones often pass naturally through the urinary tract with increased fluid intake. Drinking 2–3 liters of water daily helps dilute the urine, making it easier for small stones to pass. Pain can be managed with over-the-counter medications like ibuprofen or prescribed painkillers.

For larger or symptomatic stones, medical intervention may be necessary. Medications such as alpha-blockers (e.g., tamsulosin) can relax the muscles in the ureter, aiding stone passage. Uric acid stones may dissolve with urine alkalinization using potassium citrate or sodium bicarbonate.

Advanced treatments like spinal cord stimulation and regenerative therapies, including platelet-rich plasma (PRP) and stem cell therapy, are also offered in some cases. Patient education on ergonomics, posture, and lifestyle modifications is integral to long-term pain management. With a multidisciplinary approach, spinal pain management services aim to reduce pain, restore mobility, and help patients regain their daily routines.

INTERVENTIONAL PROCEDURES

Interventional Procedures

Interventional procedures are medical procedures that are performed using minimally invasive techniques to diagnose or treat various conditions. These procedures often involve the use of specialized instruments, imaging guidance, and small incisions or catheter insertions. They are typically performed by interventional radiologists, cardiologists, or other specialized physicians. Common interventional procedures include angiography and angioplasty, where angiography involves injecting a contrast dye into blood vessels to visualize their structure and identify abnormalities, while angioplasty is used to widen narrowed or blocked blood vessels using a balloon catheter to restore blood flow. Stent placement is another commonly performed procedure in which small mesh tubes are inserted into narrowed or blocked blood vessels to keep them open, particularly in patients with coronary artery disease, peripheral artery disease, and other vascular conditions. Biopsy procedures involve removing a small sample of tissue for examination and diagnosis, often using imaging guidance such as ultrasound or CT scans for precise targeting. Radiofrequency ablation utilizes heat generated by radiofrequency energy to destroy abnormal or cancerous cells and is frequently used for tumors of the liver, kidney, lung, or bone. Embolization is a technique used to block or reduce blood flow to specific areas by injecting embolic agents into blood vessels, making it effective for treating bleeding, tumors, and vascular malformations. Vertebroplasty and kyphoplasty are specialized procedures designed to stabilize fractured vertebrae by injecting bone cement into the affected spinal bones, thereby providing support and pain relief. Chemoembolization and radioembolization deliver chemotherapy drugs or radioactive particles directly into tumors or the blood vessels supplying them, offering targeted treatment for liver cancer and certain metastatic tumors. Drainage procedures are performed to remove fluid or pus from body cavities or abscesses, often with the assistance of image-guided catheter placement. Additionally, ablation procedures for arrhythmias, including radiofrequency ablation and cryoablation, are used to treat abnormal heart rhythms by selectively destroying abnormal electrical pathways within the heart.

THERAPEUTIC ENDOSCOPY

Therapeutic Endoscopy
The role of therapeutic endoscopy in current gastroenterology is very important. Therapuetic endoscopy is useful in treatment of gastrointestinal bleeding. Endoscopic control of gastrointestinal bleeding includes the following procedures of haemostasis techniques: photocoagulation, electrocoagulation, thermocoagulation and injection method. Owing to these procedures mortality has significantly decreased. Endoscopic hemostasis eliminates the risk of surgery, is less expensive and better tolerated by patients. Colonoscopic polypectomy is a widely used technique. By removal of polyps the incidence of colon cancer can be decreased. The "hot biopsy" forceps can be used to excise polyps of up to 6 mm. Larger polyps can be removed safely by snare electrocautery and retrieved for histologic study. Endoscopic retrograde cholangiopancreatography has a therapeutic application designed to cut the sphincter of Oddi fibers of the distal common bile duct, what is indicated currently in choledocholithiasis and papillary stenosis with ascending cholangitis, acute gallstone pancreatitis. Endoscopic sphincterotomy in now an established procedure that is indicated in patients with common bile duct calculi. Endoscopic decompression of the biliary tree - dilatation benign structures of the biliary tree with baloon catheters and placement an internal endoprothesis allows the nonoperative decompression and significant palliation for patients with obstructing tumors.

EMERGENCY ENDOSCOPY FOR GI BLEEDS

Emergency Endoscopy for GI Bleeds
The diagnosis of the cause of upper G.I bleeding is notoriously difficult and there is no substitute to actual visualization of the source of bleeding. The value of fibreoptic endoscopy, within 24 hours of upper gastrointestinal bleeding, is well known. It helps in the diagnosis of acute erosive and superficial mucosal lesions of the oesophagus, stomach and duodenum which are often missed even with double contrast barium studies. The present study was undertaken to determine the various causes of upper G. I. bleeding and compare the accuracy of clinical diagnosis with the endoscopic findings.

COLONOSCOPY

Colonoscopy

A GI Colonoscopy is an advanced diagnostic and preventive procedure performed by a Gastrointestinal (GI) specialist to examine the inner lining of the colon and rectum. It is commonly used for colorectal cancer screening, detection and removal of polyps, evaluation of gastrointestinal bleeding, chronic diarrhea, constipation, abdominal pain, inflammatory bowel disease (IBD), and other digestive disorders. Colonoscopy enables early diagnosis and timely treatment of colorectal conditions, helping maintain optimal digestive health and prevent serious gastrointestinal diseases.

UPPER GI ENDOSCOPY

Upper GI Endoscopy
An upper endoscopy is a procedure used to visually examine your upper digestive system with a tiny camera on the end of a long, flexible tube. A specialist in diseases of the digestive system (gastroenterologist) uses an endoscopy to diagnose and, sometimes, treat conditions that affect the esophagus, stomach and beginning of the small intestine (duodenum). The medical term for an upper endoscopy is esophagogastroduodenoscopy. You may have an upper endoscopy done in your doctor's office, an outpatient surgery center or a hospital

RADIOFREQUENCY

RADIOFREQUENCY
Radiofrequency (RF) therapy, also called radiofrequency skin tightening, is a nonsurgical method of tightening your skin. The procedure involves using energy waves to heat the deep layer of your skin known as your dermis. This heat stimulates the production of collagen. Collagen is the most common protein in your body. It creates the framework of your skin and gives your skin its firmness.

EPILEPSY SURGERY

EPILEPSY SURGERY
Epilepsy surgery is a procedure that removes an area of the brain where seizures occur. Epilepsy surgery is most effective when seizures always occur in a single location in the brain. Epilepsy surgery is not the first line of treatment. But it might be an option when at least two anti-seizure medicines have failed to control seizures. You might need several tests before surgery to find out whether epilepsy surgery is an option and what type of surgery to do.

It may be accompanied by vomiting, nausea, eye pain, or light sensitivity in some cases. Migraine attacks cause excruciating headaches that can last for hours or even days. The cause of a migraine is unknown; however, it is linked to unusual brain activity. The unusual changes in the brain have an impact on the normal functioning of the nervous system and result in a chemical imbalance in the brain. The following are the primary migraine triggers: Stress Hormonal imbalance Diet Physical ailments Medications Environmental triggers Furthermore, there is no single treatment that can completely eliminate migraines. The majority of available treatments are aimed at alleviating symptoms and preventing migraine attacks from occurring in the first place.

RESPIRATORY PROBLEMS

Respiratory Problems
Respiratory illness is a common problem in metropolitan cities. Certain factors and behaviours, such as smoking, air pollution, and environmental issues can contribute to respiratory diseases. Other causes include autoimmune diseases such as rheumatoid arthritis and genetic defects. Asthma, chronic obstructive pulmonary disease, cystic fibrosis, chronic bronchitis, tuberculosis, lung cancer, pulmonary edema, and pneumonia are examples of common respiratory diseases. Early symptoms of respiratory diseases include wheezing, difficulty breathing, chest pain, chronic cough with or without blood, and excessive mucus production. These symptoms are warning signs that necessitate immediate medical attention, so consulting a doctor is strongly advised.

HEART PROBLEMS

Heart Problems
High blood pressure is usually caused by the narrowing of the arteries caused by the buildup of a sticky substance called plaque. Plaque forms as a result of excess cholesterol in your blood, which is typically caused by a diet high in certain types of fats and low in fibre. As plaque accumulates along the artery walls, the arteries become less flexible and less capable of efficiently transporting blood. This is referred to as artery "hardening." High blood pressure can also be caused by smoking, obesity or being overweight, excessive drinking or other substance abuse, and being physically inactive. Blood pressure is also more common in the elderly. Various medications can be used to keep blood pressure under control. Quitting smoking, reducing unhealthy fats, and consuming more fibre along with regular exercise are all important lifestyle changes. It's critical to have your blood pressure checked on a regular basis to ensure that your treatment is working. High blood pressure usually causes no symptoms until serious damage occurs, thus making regular checkups all the more important.

HEART PROBLEMS

Heart Problems
Despite having a chronic cardiac condition, you can live a healthy, happy life if you have the right team in place, the right medications and devices prescribed as needed, and the right lifestyle changes. Symptoms can appear suddenly or gradually, depending on the cause. A very mild condition may not have any visible symptoms at all. It is critical to recognize the signs as soon as they appear and to treat the condition as soon as possible with the assistance of your cardiac healthcare team. Treatment differs from one patient to the next.

Diabetes Management
We specialize in helping patients manage their diabetes so they can stay healthy. Diabetes mellitus is a metabolic disorder characterized by high levels of blood glucose (blood sugar). Diabetes causes high blood sugar levels because the body does not produce enough insulin (the hormone that converts nutrients into energy), or because the cells do not respond to the insulin that is produced. Diabetes, if left undiagnosed and untreated, can lead to cardiovascular disease, kidney damage, stroke, amputations due to diabetic foot conditions, and blindness due to diabetic retinopathy. We have the knowledge and experience to accurately diagnose and effectively manage diabetes at our clinic. Our diabetes specialists assess and care for people with all types of diabetes and diabetes-related issues. Furthermore, our Diabetes Management Program provides comprehensive diabetes education to patients and families.

SORE THROAT

Sore Throat
Sore throats are common in children and can be painful. However, a sore throat that is caused by a virus does not need antibiotics. In those cases, no specific medicine is required, and your child should get better in seven to ten days. In other cases, a sore throat could be caused by an infection called streptococcal (strep throat). Strep cannot be accurately diagnosed by simply looking at the throat. A lab test or in-office rapid strep test, which includes a quick swab of the throat, is necessary to confirm the diagnosis of strep. If positive for strep, your pediatrician will prescribe an antibiotic. It's very important that your child take the antibiotic for the full course, as prescribed, even if the symptoms get better or go away. Steroid medicines (such as prednisone) are not an appropriate treatment for most cases of sore throat. Babies and toddlers rarely get it strep throat, but they are more likely to become infected by streptococcus bacteria if they are in child care or if an older sibling has the illness. Although strep spreads mainly through coughs and sneezes, your child can also get it by touching a toy that an infected child has played with.
